You are voluntarily electing to undergo the treatment(s)/procedure(s) after the nature and purpose of the treatment(s) has been explained to you, along with the risks and hazards involved.

Although it is impossible to list every potential risk and complication, you should be informed of possible benefits, risks, and complications. Also recognize that there are no guaranteed results and that independent results are dependent upon age, skin condition, and lifestyle and that there is the big possibility you may require further treatments of the treated areas to obtain the expected results at an additional cost.

Understand the post-treatment home care instructions provided by your esthetician. Understand how important it is to follow all instructions given to you for post-treatment care. In the event that you may have additional questions or concerns regarding your treatment or suggested home product/post-treatment care, you will consult the esthetician immediately.

You must also, to the best of your knowledge, give an accurate account of your medical history, including all known allergies or prescription drugs or products you are currently ingesting or using topically.

You must read and fully understand the agreement(s) pertaining to the treatment you will be receiving and all information detailed above and in the material provided to you by your esthetician. Understand the procedure and fully accept the risks.  Before your procedure, it is your responsibility to make sure that all of your questions have been answered to your satisfaction and you are consenting to the terms of the agreement(s). You will sign documentation stating that you do not hold Lucy Hart Ink and or the esthetician, responsible for any of your conditions that were present, but not disclosed at the time of the skin care procedure, which may be affected by the treatment performed today. 

Lastly, as an adult, you must do your due diligence before agreeing to have any procedure on your face.  You know your skin better than anyone so it is your responsibility to research, ask and fully understand the procedure prior to signing the consent form that will be handed to you before your procedure.

After Care Basics

Your Esthetician will provide specific after care instructions for you to follow, but regardless of what skin care procedure you receive, there are basic instructions to keep in mind.

Aerobic exercise or vigorous physical activity should be avoided for the first 48 hours.

Direct sunlight exposure is to be completely avoided immediately following the treatment (including any strong UV light exposure and tanning beds) . Depending on your procedure, staying out of the sun for at least a week may be recommended.  If some sun exposure cannot be avoided, first apply sunscreen with an SPF of 30 or greater. Sunscreen (with at least an SPF of 15) should become a part of your daily skin care regimen as your skin will become more sensitive to the sun as a result of this treatment.

Do NOT apply any type of glycolic acid or exfoliation products as this can severely damage or irritate the skin during the entire healing process.

DO NOT peel, rub, or scratch your skin at any time, whatsoever. This WILL cause damage and compromise your results as well as possibly cause severe scarring.

If you experience painful areas of the face, contact your skin care therapist immediately, especially if you are prone to cold sores. Any blisters that form will need to be reported immediately.  And you may need to seek medical advice from a doctor.
